An affidavit is simply a sworn statement. There are several different forms available:

Affidavit of Parental Travel
Affidavit of Paternity
Affidavit of Support is used to show the ability of a servicemember to support another individual who is visiting here on Island, based on the servicemember’s income.
Affidavit of Lost Title is used when a servicemember loses their vehicle title. They will need this affidavit before receiving a duplicate title.
Affidavit of Competency to Marry is common in Okinawa. Servicemembers must have this before they can marry a U.S. citizen in Okinawa, or any non-U.S. citizen anywhere.
Affidavit of Presentation is used when a client needs to make copies of an original document. This affidavit is used when trying to get a Social Security Number, or and ITIN. We create these instead of doing "Certified True Copies."
Affidavit of Statement is used for a client that needs to make a statement and swear to it that the statement is true.
Affidavit of Sole Ownership is used for a Marine or Sailor to swear to the fact that they are the sole owner of an item.
Affidavit of Okinawa Residency is used to state that a Marine or Sailor was living in Okinawa during a certain period of time.
Affidavit of Parental Travel is used to permit either the father or the mother to travel with a child to the States or to another country.
Affidavit of Paternity is used to prove that a client is the father of a child. Any client who wishes to execute one MUST meet with an attorney first.
